Below is a list of PASAs approved across the State of Colorado. If you see a provider that you would like to receive services from and they are not on the PASA List, please contact your Case Manager.

Interested in becoming a Program Approved Service Agency (PASA)?
Please first apply through the Health Facilities and Emergency Medical Services Division (HFEMSD) within the Colorado Department of Public Health and Environment (CDPHE).

Upon your organization's PASA approval, A&I Avenues will be contacted directly by the Office of Community Living, Department of Health Care Policy and Finance (HCPF). A&I Avenues PASA Quality will then email your organization additional information and next steps with how to move forward and begin to serve persons through A&I Avenues.
