Family Support Services Program

A&I Avenues' Family Support Services Program (FSSP) is here to assist families who provide at-home care to family members with intellectual and developmental delays or disabilities. Our services are designed to ease the financial and emotional strain of having a family member with a disability in the home. Additionally, we provide grants to eligible families who need extra funds to help pay for items and services for their family members in need when not covered by other resources.

What is the Family Support Services Program?

Family Support provides grant funding to eligible families as well as access to Family Support Navigators who can support with questions about the program and funding available, community resources and supports, waiver referrals, and more. Financial grant funding is intended to support the extraordinary costs of caring for a family member with intellectual or developmental delays or disabilities when those needs are not covered by other resources. Grant funding is distributed at designated times through the fiscal year and based on funding available.

 

Who is eligible for services?

Family Support Services requires the individual with an intellectual or developmental disability or delay to be living with family in Boulder, Broomfield or Gilpin County. Eligibility requirements include an intellectual or developmental delay if under the age of 5, and an intellectual or developmental disability determination if over the age of 5. When can I apply?

The Family Support Program operates on a fiscal year basis from July 1st to June 30th of the following year. Family Support Plans are automatically sent to eligible families at designated times through the fiscal year.

 

How can I apply?

Once an individual is found eligible for Family Support Services, the family will be sent a Welcome Letter packet detailing next steps. In order to apply for Family Support funding, a Most in Need (MIN) assessment is required to be submitted, which will be sent in the initial Welcome Letter packet as well as to all enrolled families on an annual basis.

What is a Family Support Navigator?

A Family Support Navigator is similar to a Case Manager. They are available to support with program documents and processes, provide information and support navigating community resources and can support with waiver referrals. Family Support consumers do not have assigned Navigators and are welcome to contact any Navigator on the team at anytime for support.

Starting the FSSP Eligibility Process

  1. Please reach out to the A&I Avenues Intake Department using our contact form to complete the eligibility requirements.

  2. Once eligibility for Family Support has been confirmed, a member of the A&I Avenues FSSP team will send a welcome letter with a Most In Need (MIN) assessment attached. Once the MIN assessment has been returned, the Family Support team will follow up with available funding information, guidance on how to apply and next steps.
